When the top vertebrae in the cervical spine are out of alignment, the rectus capitis posterior minor, or RCPM, muscle spasms. This small muscle is connected to a tendon that slips between the base of the skull and the upper neck and attaches to the dura matter, which is sensitive to pain. When the RCPM muscle spasms and the tendon tugs at the dura matter, a headache typically occurs.

Referred pain from trigger points in the Sternocleidomastoid (SCM), or levitator, muscle located on the side of the neck can also cause tension headaches. These headaches are especially common in those who have suffered whiplash injuries because of the muscle damage sustained in the neck area.
